Below are four things you MUST be absolutely clear on before you fill out the client verification form.

1) Fill out the form truthfully, completely and honestly. We are not idiots, intentionally dishonest, deceptive or incomplete applications will be declined. Verification is absolutely NON-NEGOTIABLE. Discretion and professionalism on our part is assured we will not call home numbers unless you say so. Do not attempt to deceive us by putting a cell number as your home phone.

2) DO NOT share your CLIENT CODE with anyone other than Fantasy's phone operators or you will be BANNED permanently! In your approval e-mail you will be given a special number to call. DO NOT give out your client code to other Providers, (we don't care what they say) your friends or ANYONE. Doing so will get you BANNED !! - Can we make this any clearer ? -

3) Should an emergency happen as they sometimes do at the last minute, we ask for a reasonable amount of time to cancel the appointment. Failure to cancel your appointment within a reasonable amount of time MAY result in your not being permitted to use our or the Provider whom we are screening for services again.

4) Until you have completed an appointment with a Fantasy's Girl or a member of our partnership network we will not give you a provider reference to other "non-Fantasys" providers. Your approval e-mail will contain instructions on what to do once you've completed your appointment.
